Title PROFESSIONAL SUPPORT I – AWAKENINGS, PRN
Position Type: PRN, between hours of 8a-4p; must work 8 hours per month
Position Summary:

The role of the Professional Support Staff I is to help individuals with disabilities to live/work as independently as possible, reach their goals, develop skills and provide support when required. This position works as a part of a team of staff who will continually assess and meet the persons served needs. This entry-level position requires a HS Diploma/GED; one year of working with developmental disabilities or a similar population is preferred. Position Type: Salaried/Exempt
Position Summary:

Will have oversight and responsibility for
overall operation of assigned department, which includes both people and
financial resources.  Will be responsible
for ensuring appropriate services are delivered to individuals served in the

Title RN II – CASE MANAGER – FT
Position Type: FT 40 hrs/week; Mon. – Fri. flexible hrs
Position Summary:

Will practice as an RN in compliance with COMAR regs including the Nurse Practice Act and Developmental Disabilities regs. Serves as the Case Manager/Delegating Nurse for an assigned group of individuals, leading a team which may consist of an LPN and Medical Assistant. Will assume responsibilities of delegation to unlicensed and licensed staff providing services to assigned individuals. Will be on-call for caseload and participate in an on-call program for Healthcare Services. Also responsible for training agency staff and outside agencies in the areas of CMT and C.N.A. and other trainings as needed. Researches, develops and implements new training programs as assigned.

Qualification Requirements

Must be licensed to practice in Maryland. Must have two (2) years of clinical nursing experience or six months experience as an RN I, plus one year of working with developmental disabilities or a similar population preferred. Must have a valid driver’s license along with three (3) years of driving experience. Job Specific Competencies

Collaborates with the Director of Healthcare Services to maintain and update a system of standard educational training plans to meet the needs of the individual.

Ensures that individuals receive appropriate medical services (medical, dental and psychiatric appointments; lab and diagnostic studies; medical and dental procedures, etc.) in a timely manner.

Ensures that immunizations remain current for assigned individuals.

Ensures that the Authorization for Use of Mechanical Supports are completed annually and where there is a change in supports used while ensuring that Instructions for Use of Mechanical Supports remain current.

Completes documentation of the assessments to ensure that completed assessments are available in the home within two (2) weeks of the assessment date.

Completes annual reviews for individuals prior to annual IP including the Annual Nursing Summary of the individual’s history during the past year.

Ensures that the Health Maintenance Plans address the current needs of each individual, including developing and implementing new plans for change in condition or an acute issue.

Ensures that 90-day authorizations for restraints/ sedation for medical purposes are completed

Ensures that nursing recommendations for over-the-counter topical products are completed

Responds to on-call pages in a timely manner and provides sound nursing advice

Assist the nursing team with unassigned routine duties (taking calls for other nurses, providing services to individuals that are not part of assigned caseload, vaccine clinics, assisting with pharmacy deliveries, etc.)

Assists with respite admissions and organizes and monitors pertinent records for same.

Responsible for training agency staff and outside agencies in the areas of C.N.A. and CMT, as well as other trainings as needed.

Will assist persons served in the completion of any/all of their job duties that are necessary for the maintenance of adequate production levels
Will insure clear communication between persons served and employers in matters relative to the person served’s employment
Will be responsible in the procurement of career opportunities under the supervision of the designee
